Job description

With training, demonstration of competency, and with the supervision of the RN, Case Manager, performs patient care services according to the individual patient’s treatment plan. Examples include: apply simple non-sterile dressings; assist with bowel and bladder programs; obtains vital signs; performs PT/INR testing, etc…

Assists patient in maintaining personal hygiene according to the individual patient’s treatment plan. In so doing:

Takes routine care of patient’s hair and may shampoo as instructed

Assists patient with care of mouth, dentures, and nails
-
Assists patient with bathing
-
Makes observations for a change in patient status including skin condition changes (i.e. pressure ulcers)

Performs routine housekeeping tasks in order to promote a safe and comfortable environment for the patient such as changing bed linens, completing laundry/dishes, and preparing simple meals

Observes and reports to RN, Case Manager, any changes in patient’s physical or mental condition. Completes documentation in a timely and accurate manner. Coordinates with other professional staff when there is a need to revise the care plan.

Accompanies patient when they are permitted outside the home. Promotes mental alertness of the patient through conversation and activity. Utilizes infection control practices with the delivery of patient care services.

Attends/participates in home care and hospice teams meetings, quality improvement programs, and patient care conferences.

Performs all patient care services in accordance with state regulations and follows the appropriate state scope of practice LNA guidelines.

High school graduate or equivalent

PURPOSE
Provides personal care and other supportive services to individuals in their homes. Assists the patient to live with dignity and safety in an independent community setting setting or within a regional facility. Carries out the duties assigned in order to increase the level of comfort and to maintain personal hygiene and a safe, healthy environment for the patient. Provides for temporary respite and relief from total care responsibilities for the primary caregiver.
