LPN needed full time for The Guidance Center Primary Care Clinic. Looking for an experienced individual (primary care experience preferred) who possesses a positive team player attitude. Will be responsible for 5 day/week day shift, Monday - Friday.

Essential Functions:
+ Interviewing patients and recording a thorough past medical history and family
history
+ Obtaining patient vital signs and escorting patients to their rooms
+ Administering medications, vaccinations, and other injections prescribed by the
provider
+ Observing and recording patients’ conditions and reactions to medications and
treatments, reporting any concerns to the provider in office
+ Formulation of the plan of nursing care and evaluation of the client's
individualized response to the care rendered
+ Communicate with pharmacies and insurance companies to perform Prior
Authorization requests for prescribed medications/supplies when needed. This
includes compiling all needed information to support medication/supplies and
providing it to the insurance for their review
+ Performing physical, psychological, and social assessment, including substance
use screening tools
+ Making an educated judgment about a potential or actual health problem with a
client and relaying it to the appropriate provider
+ The responsibility and accountability for client health teaching and health
counseling, which promotes client education and involves the client's support
system, when available, in accomplishing health goals
+ Teaching and counselling clients or family members on health-related issues
while collaborating with other healthcare professionals
+ Monitoring, addressing, and documenting the symptoms and changes in clients
conditions
+ Operating and using medical equipment in the clinic for patient testing or care
+ Assisting Providers with any medical procedures
+ Other job duties can include performing point-of-care testing, collecting
specimens, and scheduling appointments
+ Interview patients about previous healthcare providers and health maintenance
performed and obtain release of information for those providers/facilities and fax
to them for request of records
+ Medication reconciliation at every visit to be sure that current medication list is
accurate
+ Calling test/lab results to patients as instructed by provider
+ Communicate with patients via telephone and portal, reporting necessary
concerns or questions to the appropriate provider
+ Daily temperature monitoring of medication refrigerators and documentation of
temperature on designated temperature logs
+ Reporting low inventory of needed supplies/medications to clinic director
+ Assist with inventory of clinic samples, checking for and removing expired
samples from inventory, signing out medications for patients as directed by the
provider, and logging in new medications as they are received
+ Monthly checks of clinic supplies to check for and remove any expired supplies
from inventory and/or exam rooms
+ Keeping exam rooms sanitized and stocked with necessary supplies
+ Performing Prescription Monitoring Program (PMP) reports on patients when
indicated for provider’s review.

Requirements:
+ License practical nursing certification and ability to pass a background check.
+ Experience with clinical procedures, such as: a specimen collection and
diagnostic testing.
+ Ability to record, monitor, and report a patient’s condition to professional
personnel.
+ Knowledge of healthcare practices, standards, and medical terminology.
+ Advanced record keeping abilities and proficiency in related software, such as
Credible and Elation EMR charting systems.
+ Strong interpersonal and communication skills, both verbal and written
+ Ability to maintain composure to respond to emergencies and distressing
situations.
+ Strong ability to maintain safety, quality, and infection control standards.
+ Strong knowledge of medical procedures, equipment, and medical facility’s
policies and procedures.
+ Ability to perform basic nursing duties and responsibilities.
+ Ability to work in stressful environments and under pressure.
+ Excellent time management and organizational skills.
+ Ability to work independently and as part of a team.
+ Must be willing to perform and pass check off procedures with/to the
practitioner's satisfaction.

Job Type: Full-time

Pay: $16.00 - $18.00 per hour
