Job description
Under general direction of the Director of Population Health and Equity and working in cooperation with other agency departments, the Population Health and Equity (PHE) Manager is responsible for the implementation of the Alliance’s population health assessments, strategy and evaluation. In addition, the PHE Manager oversees the execution of the Alliance’s population health, health education and health equity programs, supervises PHE staff, and ensures compliance with state and federal regulatory and accreditation requirements concerning population health, health education, and health equity.
Principle responsibilities include:
- Effectively manage the development and implementation of the Alliance population health assessment, strategy, and evaluation.
- Oversee the creation, implementation, and evaluation of the health education, health equity, and population health programs and services.
- In coordination with Case Management, implement a disease management program to support Alliance members with condition self-management.
- In conjunction with department leadership ensure continuous regulatory and accreditation readiness for DMHC, DHCS, and NCQA.
- Assure that a wide variety of health education programs are available for Alliance members and meet the needs of the Alliance's culturally and linguistically diverse membership.
- Lead the development of member communications and interactive health promotion, including the use of new technologies.
- Assure health education materials meet the literacy, cultural, linguistic, regulatory, and clinical standards appropriate for the target audience.
- Collaborate with providers, clinics, and public and community-based agencies and programs to implement health and wellness interventions for members.
- Educate community partners about Alliance services, benefits, insurance coverage and clinical priorities, including community presentations.
- Maintain professional contact with community partners and the Local Health Plans.
- Lead the organization and implementation of focus groups, advisory groups, and assessments to ensure member input into the health programs of the Alliance.
- Take primary responsibility for the Member Advisory Committee, including recruitment, retention, agenda development, and oversight of meeting logistics.
- Prepare and present regular reports for various committees and subcommittees of the Board of Governors.
- In conjunction with QI Leadership develop program budgets and ensure fiscal responsibility.
- Ensure regulatory compliance, approvals, and fiscal responsibility for member incentive programs.
- Develop, implement, and monitor contracts with vendors.
- In conjunction with QI leadership review and revise as necessary, all QI policies and procedure documents.
- Maintain updated departmental standard work documentation and workflows.
- Complete other duties and special projects as assigned.
ESSENTIAL FUNCTIONS OF THE JOB:
- Develop, implement, and evaluate health education, health equity and population health programs.
- Maintain current knowledge of healthcare industry best practices and managed care regulatory requirements.
- Data collection, measurement, maintenance, and analysis.
- Participate in organizational and community level planning/support.
- Perform writing, administration, analysis, and report preparation.
- Communicate and coordinate with Alliance departments and outside agencies.
- Demonstrate skills in public speaking, writing, and verbal communication.
- Lead and participate in internal and external committees and meetings.
- Comply with the organization's Code of Conduct, all regulatory and contractual requirements, organizational policies, procedures, and internal controls.
PHYSICAL REQUIREMENTS:
- Constant and close visual work at desk and computer.

- Constant verbal and written communication with staff and other business associates by telephone, email, video conference, or in person.

MINIMUM QUALIFICATIONS:
EDUCATION OR TRAINING EQUIVALENT TO:
- Bachelor’s degree or equivalent in health sciences or related field.
- Master of Public Health (MPH) in Health Education or Community Health from an accredited program preferred.
- CHES/MCHES certification is preferred.
MINIMUM YEARS OF ADDITIONAL RELATED EXPERIENCE:
- Five years of program management experience in the field of population health, health education, health equity, or related field.
- Two years of supervisory experience.
- Two years of health programs experience with Medicaid or similar populations preferred.
- Two years of experience with NCQA accreditation preferred.
- One year managed care experience preferred.
SPECIAL QUALIFICATIONS (SKILLS, ABILITIES, LICENSE):
- Ability to work cooperatively with diverse groups including low-income populations, health care providers and community partners.
- Knowledge of health education and population health principles and methods.
- Ability to organize, promote, conduct public meetings and education sessions.
- Ability to meet deadlines and adjust to changes in policies, procedures, and priorities.
- Excellent verbal and written communications skills.
- Knowledge of and experience with the community and other resources.
- Must be organized, detailed oriented, and can work in cooperation with others.
- Solid critical thinking and problem-solving skills.
- Ability to utilize sound judgment and promptly report potential risks.
- High degree of independence, flexibility, initiative, and commitment.
- Ability to analyze and use health data to monitor and prioritize work
- Strong proficiency in Microsoft Office suite, including Word, Excel, and PowerPoint, and Adobe Acrobat Pro.
- Must handle PHI and maintain confidentiality at all levels.
- Knowledge of NCQA accreditation requirements preferred.
- Knowledge of Medi-Cal and other entitlement programs preferred.
Salary Range: $113,669.74 - $170,504.61/Annually
