Job Title: Patient Advocate
Report to: Director of Human Experience
FLSA Status: Non-Exempt
Job Summary: Responsible of the On-Boarding, engagement, and loyalty of patients in the practice.
The Patient Care Advocate is an essential role within InnovaCare Health. This position will assist in the planning and implementation of outreach strategies. At the heart of our organization is the strong desire to help our patients get access to healthcare. The Patient Care Advocate is key to us accomplishing this objective.
This position requires an ability to engage and educate our patients and a strong ability in building relationships. The Patient Care Advocate must also have strong organization skills, a desire to help people and must possess a welcoming personality.
Responsibilities:
Provide outreach services to engage patients and educate them about InnovaCare clinics and services. Retain supply of brochures and other materials necessary for outreach activities. Reach out to and receive calls from established and prospective patients. Understand Medicare and insurance basics and guide patients with how benefits work at InnovaCare. Schedule patient appointments ensuring the patient experience is flawless. Provide outreach, and education to individual patients and groups in a wide variety of settings and to all cultural and ethnic groups in a way that will promote behavioral changes. Contact new patients referred to InnovaCare to introduce them to our clinic staff and provide tours. Locate patients and communicate information to them as needed, with appropriate follow-up to the proper department within InnovaCare. Arrange transportation and refer patients to transportation carriers if needed. Maintain confidentiality regarding patient and/or family in accordance with professional and departmental standards. Adheres to ethical and legal standards of practice. Precise and complete data collection, timely data input, program evaluation and development, and reporting of results to leaders. Assist InnovaCare with monthly monitoring of department performance. Support organizational efforts to drive patient experience and loyalty initiatives to ensure organizational alignment Conduct frequent interviews and surveys to patients to identify and address opportunities within the medical practice. Support Online Reputation initiatives to promote patient participation. Plans and coordinates social and educational activities Tracks event attendance develops outreach strategies, and generates new patients for office ( T65 and or age-in). Maintains relationships with assigned insurance agents and vendors (area sales manager and / or Broker Relations cord.) Provides office VIP tours as needed. Provides new patients with new patient Welcome Package prior to or during first appointment. Conduct New Member welcome events as needed. Assists patients in completing paperwork and provide patient information on the Patient Portal. Completes patient satisfaction survey Acts as patient concierge and regularly interacts with patients waiting in lobby and at the Activity Center. Resolves customer service issues as presented to them. Maintains Activity and Resource Center on· a daily basis to ensure all areas are clean and organized.

Qualifications and Education Requirements
- High School graduate, with three years of experience where the major responsibility was in any one of the following categories: health care, economic services, counseling, sales, medical billing, or processing insurance claims.
- Clinical experience a plus.
- Medical Assistant a plus.
- Valid Florida Driver’s License and has reliable/accessible transportation.
- Bilingual Spanish/English preferable.
- Must have knowledge of community.
- Must exemplify a professional demeanor.
- Possess empathy with the target population and must be culturally sensitive to all racial/ethnic groups.
- Ability to communicate effectively verbally and in writing.
- Ability to establish and maintain effective working relationships with supervisors and peers.
- Must be a self-starter.
- Ability to work independent of constant supervision.
- Ability to prepare and maintain detailed records and notes.
- Be computer proficient.
- Must be able to prioritize work and be flexible to changes to any processes or work assignments.
- Must be able to lift up to 25 pounds.
