Ability to deliver patient care in a manner that is appropriate to the demographics and/ or specific needs of the patient population served whose cardiovascular needs range from basic to complex. Creates a therapeutic environment for patient/family care. Serves as a resource for all clinical related areas.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Under the general direction of the Practice Administrator and in accordance with standards of care and practice established by the Nursing Organization and the specific department; the rules, regulations and/or standards established by the State Board of Nursing, the Joint Commission, and appropriate specialty organizations relevant to the patient population under care, the incumbent performs the following essential functions:

Patient Care:

Assessment:

Interviews the patient and/or family to obtain a comprehensive history and to obtain data about patient’s development status, behavior, cultural, and health concerns.

Examines the patient to obtain objective physical data.

Reviews clinical data from diagnostic studies and previous records.

Completes a physical and psychosocial assessment based on interview, examination and review of clinical data.

Based on assessment, identifies nursing diagnoses/patient care problems.

Documents data and assessment on appropriate form according to instructions.

Collaborates with the multidisciplinary team to address patient needs.

Reassesses patient status at established time frames and as indicated by the patient’s status.

Reports status and changes in status as appropriate.

Assesses potential for development of crisis situations and asks for assistance.

Performs telephone triage appropriately assessing signs and symptoms, potential risk for complications and necessity of immediate interventions.

Planning:

Develops an individualized approach to deliver nursing care to the patient and family, including age-specific developmental needs and incorporating their input and preferences whenever possible.

Establishes goals with the patient and the family.

Collaborates with the multidisciplinary team to develop an integrated plan of care.

Initiates discharge planning as an integral part of the plan of care at the time of admission and updates status of the discharge plan on a continuing basis.

Documents plan of care appropriately.

Implementing Plan of Care:

Coordinates and implements the medical diagnostic and therapeutic orders.

Performs nursing care tailored to the individual’s needs.

Provides for patient’s dignity during provision of care.

Ensures a calm, supportive and therapeutic environment for patients and their families, serving as an advocate when appropriate.

Assesses the patient’s response to and understanding of care provided.

Documents implementation of care appropriately.

Reports patient’s response to care as indicated.

Completes all required forms which support the continuum of care.

Provides education based on patient/family’s assessed needs.

Participates in the care of patients with complex problems.

Participates in complex procedures, including pacemaker and defibrillator interrogation and reprogramming and transesophageal echocardiogram procedures.

Interprets and manages protime results accurately within approved written guidelines. Consults with a cardiologist when indicated to make appropriate adjustments. Contacts patients with results within 24 hours.

Monitors that patients complete testing procedures. Reviews test results. Contacts patients with results and recommendations, after collaborating with a cardiologist, within seventy-two hours.

Evaluating Plan of Care:

Reviews and updates the plan of care in collaboration with the patient/family and other multidisciplinary team members.

Documents progress toward expected patient outcomes.

Reassesses patients’ abilities and incorporates anticipated needs into the plan of care.

Documents all treatment and patient/family interactions/interventions in the medical record.

Adheres to safety, organizational and ethical standards including:

Organizational safety standards.

Company and Nursing Organization policies, guidelines and procedures.

Infection Control standards.

Standards of confidentiality, protection of patient rights.

Professional Development:

Promotes positive customer relations.

Participates constructively in staff meetings.

Participates in and supports Quality Management.

Conforms to the organization attendance requirements.

Demonstrates behaviors which promote communication and collaboration.

Utilizes the self-evaluation process for professional growth

Identifies self needs then increases skills and knowledge.

Attends mandatory in-services.

Participates in department projects/committees.

Accepts constructive feedback and initiates appropriate action to correct the situation.

Identifies research as a resource in clinical practice.

Exhibits a professional approach/demeanor.

Maintains membership in a professional nursing organization or community organization.

Directs concerns through correct channels in a professional and responsible way.

Participates actively in and coordinates clinical research studies adhering to Institutional Review Board and specific research protocol guidelines.

Leadership:

Prioritizes and manages assignment effectively as evidenced by task completion in a timely manner.

Contributes to the development of staff.

Monitors the care students and/or ancillary staff provide.

Promotes peer group functioning.

Contributes to an individualized orientation.

Performs department duties independently.

Supports hospital management.

Implements unit’s delivery of care methodology.

Demonstrates cost effective use of resources.

Qualifications:

Education:

  • Graduate of an accredited school of nursing.

Experience:

  • Minimum of two years recent clinical experience in cardiac care.

Licensure/Certification:

  • Current registration in the state of New Hampshire as a Registered Nurse.
  • BLS certified. Must maintain BLS throughout employment.
  • ACLS certified. Must maintain ACLS throughout employment.
