General Job Description
The RN / LVN Charge Nurse performs a wide variety of patient care and management activities in order to facilitate the delivery of prompt, high quality and compassionate care. Diamond Neighborhoods Family Health Center’s Women & Children’s department provides pediatric & women’s health services (GYN, OB & Family Planning) to a diverse population. The Charge Nurse works under the supervision of the RN Nurse Manager and Clinic Director.
Job Roles
- Supervises a team of approximately 6 Medical Assistants & manages clinic’s nursing schedule to ensure adequate staffing.
- Keeps clinic workflow moving, focusing on productivity and efficiency, including assigning lunch times and breaks, ensuring exam room is set up, supporting provider productivity, reviewing patient problem list prior to visit with provider, and calling in refills to pharmacy.
- Assists MA staff with rooming and discharge of patients, as well as nursing care.
- Assesses walk-in patients to ensure appropriate care or referrals are given.
- Serves as a resource for dealing with the more complex patients.
- Orders supplies, medications and vaccines to ensure correct supply is on hand.
- Communicates effectively with interdisciplinary care team and support services, including Physicians & other Medical Providers, PSRs, Medical Assistants, and others.
- Supervises assigned team, including hiring, and scheduling as needed.
- Coaches and counsels staff and administers appropriate disciplinary action in a timely manner.
- Builds positive relationships, inspires others to perform at a higher level, prepares career development plans and retains an accountable workforce.
- Member of the Diamond/Oak Park clinic management team. The full-service Diamond clinic offers Family Medicine, Women & Children’s, Specialty services, Mental Health plus Vision & Dental.
- Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards. Ensures the team does so, as well.
- Consistently follows Policies and Procedures to ensure positive patient outcomes.
- Keeps site in compliance with TJC, DHS, CHDP, and Medi-Cal Managed Care requirements.
- Actively participates in other clinical initiatives, such as clinical quality outcomes, to achieve goals and metrics. Participates in & encourages team in continuous improvement activities.
- Performs other duties as assigned.
Education/Certifications/Licenses/Registrations
- California State Board LVN or RN License required.
- Graduation from a recognized LVN or RN program.
- Current American Heart Association healthcare provider CPR (BLS) certificate.
- High School Graduate (or GED equivalent).
Experience/Specialized skills (including Language)
- At least 1-2 years of supervisory experience in an ambulatory care or similar setting preferred.
- Knowledge, experience, and ability to perform clinic-based nursing tasks.
- Knowledge of child, adolescent, and adult health care issues.
- Bilingual in English/ Spanish is preferred.
- Ability to work effectively with patients from diverse social, cultural and economic groups.
- Ability to participate collaboratively with multi-disciplinary care team
- Able to maintain good working relationships with employees, coworkers, and departments.
- Strong leadership & excellent customer service skills
- Ability to work independently and proactively with a take charge attitude.
- Supervisory/management experience preferred with effective training and coaching skills.
- Basic computer literacy ability to comply with department needs (i.e., electronic medical record documentation, obtaining reports on patients, following up on appointments, etc.).
