Description

Mass General Brigham Home Care works with BWH and MGH in a collaborative partnership called Home Hospital. The program is based on our patients’ strong desire to maximize their time at home. When illness develops and is significant enough to require a hospital stay, Home Hospital is called upon to provide similar care for patients at home to avoid time in the hospital. You can think of Home Hospital as sending the hospital home with the patient. Home Hospital is more advanced than traditional home health care. Just like if a patient were in the hospital, this program would provide for several visits with our clinicians each day to treat the conditions until the patient is safe enough to be cared for by a regular clinical team.

Common conditions treated by Home Hospital include congestive heart failure, COPD, pneumonia, skin infections such as cellulitis and urinary tract infections as well as many other conditions deemed safe enough to do in the home.

In this role the RN supports the Home Hospital Program based either at BWH or MGH and works in an 8-10 mile radius surrounding the hospital treating patients in that area.

Job Summary

Full time, 9am-7:30pm-/4-10hour days

eve diff from 5pm on

Acute Care -Home Hospital Program- Boston area

every third weekend rotation-weekend diff/all hours on weekend

Provide quality, clinical services to patients in various settings in the community. Patient care is delivered within organizational philosophy, policy and standards of nursing and community health practice. The responsibilities are performed in accordance with MGB Home Care's policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications. Skilled nursing care is provided in the form of assessment, teaching, treatment, documentation and care coordination demonstrating high quality customer service, and financial awareness.

Qualifications and Experience

• Graduate of an approved School of Nursing
• Current Massachusetts license as Registered Nurse
• Minimum of one year previous nursing experience in acute, emergency or post-acute care setting required.
• Prior home care experience preferred.
• Travel required within Home Hospital geographic area

• Ability to work with various computer software applications and technologies required.
• Strong interpersonal and customer service and ability to work with a diverse population
• Ability to communicate effectively in writing, verbally and electronically
• Ability to work as a member of an interdisciplinary team of health care providers
• Ability to organize and prioritize work, and adapt to changing situations
• Ability to work independently, be self-directed and adapt to unpredictable circumstances.
