Hinds Hospital Nurse Liaison Position:

This position is dedicated to serving the hospital and its staff and those patients eligible to access their Medicare Care Hospice benefit. Through being an advocate for hospice and palliative care, developing collaborative relationships with the hospital staff and meeting the needs of patients and families, superiors care and services will be delivered. This position has two major elements. One, conducting clinical evaluations in collaboration with local hospitals, facilities and healthcare agencies, to determine hospice eligibility and perform admissions of patient referrals for both Inpatient and Outpatient Services under the direct supervision of the Director of Inpatient Hospice Services. To develop and coordinate Plans of Care for those patients receiving GIP level of care in the hospital. Second, provide needed care and support for those patients being discharged from the hospital to Hinds IPU and home care services. Work with the hospital staff to support and assist with referral development and admission activities for those patients eligible for hospice care.

• Five to seven years of nursing experience with preferably 3 in a hospice setting. Experience as an Emergency or ICU nurse preferred.
• Completed a professional nursing program accredited by the National League for Nursing (prefer completion of a baccalaureate program in nursing).
• Current California RN Licensure.
• Demonstrates the ability to function independently with strong clinical skills and critical thinking abilities as relates to patient care and staff responsibilities.
• Excellent interpersonal and communication skills.
• Creative problem solver with the ability to achieve positive outcomes.
• Exhibits sensitivity to cultural differences and has knowledge of current trends in home health care, federal and state regulation, state licensing requirements and other standards related to hospice care.
• Demonstrates ability to remain calm with staff and family members under stressful conditions.
• Maintains patient and family confidentiality within regulatory parameters and respectful of patient belongings and environment of care.
• Ability to work as a strong team player.
