The Healthcare Programs Social Worker I position is responsible for conducting in-depth Health Risk Assessments (HRAs) covering psychological, social, physical, and environmental concerns for adults with disabilities and older adults. S/he will identity the client’s needs and will follow up with the client via phone to conduct a Post HRA Outreach (PHO) to connect them to available community resources. S/he will communicate with the Health Plan if a client has further need for services not available in the community, including medically tailored meals and home safety devices.

Qualifications:

• Bachelor’s Degree in Social Work or related field
• Bilingual Spanish required
• Experience and interest in providing care to adults with disabilities and older adults
• Case management and community-based service delivery experience required
• Knowledge of community resources and the aging process
• Knowledge of safety issues when working with at-risk clients
• Experience conducting in-home visits strongly preferred
• Ability to function independently and with flexibility
• Experience with Electronic Health Records systems desired
• Must be computer literate in MS Office applications (Word, Excel, and Outlook)
• Demonstrated ability to work in an interdisciplinary community setting maintaining appropriate professional boundaries
• Culturally sensitive and non-judgmental in working with a multicultural client population
• Must pass a criminal background check (Live Scan fingerprinting) prior to beginning employment

Responsibilities/ Essential Functions:

• Conducts in-depth Health Risk Assessments (HRAs) covering psychological, social, physical, and environmental concerns in client’s homes
• Communicates with clients on the phone to connect them to available community resources
• Follows up with clients to ensure they have activated the needed resources and documents outcomes accordingly
• Maintains comprehensive and timely documentation of all client contacts and securely transmits them to Health Plans
• Communicates with Health Plans ongoingly regarding any high-risk issues or needed specialty or purchased services
• Updates the Health Plan Database to include all client outreach and contact attempts and outcomes
• Keeps track of monthly program statistics
• Other related duties as assigned
