Agency Information: Osawatomie State Hospital (OSH) is licensed by the State of Kansas to provide care and treatment for adults diagnosed with psychiatric disorders regardless of ability to pay. OSH serves individuals from 36 counties in Kansas in collaboration with 12 Community Mental Health Centers (CMHCs). The centers refer individuals for admission to the hospital through a collaborative screening process. There are currently two hospitals on the Osawatomie campus in Miami county: Osawatomie State Hospital and Adair Acute Care.

Position Summary & Responsibilities
Position Summary: The Osawatomie State Hospital is currently seeking a full time Social Worker. This position is an integral and critical part of the treatment and discharge plan for the patients at Osawatomie State Hospital. Discharge planning begins at day one of hospitalization with the patient’s goal being priority. The Social Worker Specialist is responsible for gathering and documenting information in the Social Service Assessment and Discharge plan. This plan details the decisions for the specific discharge plans. Additionally, social workers collaborate with community providers and the patient’s significant others to locate resources for discharge. Social workers are the primary liaisons to families as well as the community resources, relaying information to the team in written form as well as verbally. Social Workers coordinate placement of patient to ensure that patients are released to the least restrictive environment possible, without endangering safety. This incumbent works independently in this position, making many decisions on his/her own.
Job Responsibilities may include but are not limited to the following:
Discharge Planning: Coordinates placement of patient to ensure that patients are released to the least restrictive environment possible without endangering safety. Implements this by: 1) Utilizing the initial discharge plan as a starting point and marker of the needed services and accommodations. 2) Consulting with the treatment team, mental health center liaisons, the patient, and involved family members to help determine needs priorities. 3) Matching identified needs with community resources and developing appropriate resources if needed. 4) Initiating and periodically reviewing referrals needed to facilitate placement (financial, residential, mental health, and substance abuse support services) in a timely manner. Utilizes resources creatively and interactions are flexible.
Assessment: Collects, evaluates, and documents social history and assessment information, as assigned. Gathers information from all available sources to formulate a social history and psycho-social assessment using approved social work methods and techniques. Completes assessments based off their identified timeframes to provide the treatment team with necessary input for patient assessment and treatment planning.
Documentation: Writes or dictates progress notes to document progress made toward discharge of patients. Also documents in progress notes on assigned patients regarding interactions with patients, their involvement in the discharge process, involvement by family and significant others, Community Mental Health Center and community support services involvement, and plan for continuing social work action including continuing care planning.
Teamwork: Provides Social Work coverage identified based of needs determined by the Director of Social Services and provides input during treatment team meetings, implementation and progress regarding discharge planning. Ensures needs and resources are accurately identified and addressed in treatment planning and discharge discussions.
Qualifications
- Education: Bachelors
Licensing & Certification: Must possess a license or temporary license to practice Social Work in the State of Kansas.
Minimum Qualifications: Must possess a license or temporary license to practice Social Work in the State of Kansas.
Preferred Qualifications: Prior experience working with individuals who have psychiatric diagnosis.
Post-Offer, Pre-employment Requirements: Must complete and pass background and drug screen.
