Job Title: Care Services Social Worker

Department: Care Services & Advocacy
Reports To: Director of Care Services & Advocacy

Position Summary: The Care Services Social Worker works with the Director of Care Services & Advocacy and Care Services team to provide support to people diagnosed with ALS (or PLS), their caregivers and families. This role facilitates a higher quality of life for those living with ALS through assessment of needs and identification and referral to available, appropriate support services both in and out of the Chapter. This role offers support through regular attendance at ALS clinic(s) as a representative of the ALS Chapter and by providing education and information. This role is responsible for incoming phone calls or emails from family and community regarding emotional support, resource and referral, and requests for educational trainings in a variety of community settings. This role understands the Chapter brand promise; everything the
Chapter does supports the discovery of effective treatments and a cure for ALS.

Core Job Responsibilities:

  • Provide support and education to persons living with ALS/PLS, and their caregivers and families through phone, email, postal mail, Connection Groups, community contacts, or (occasionally) a home visit
  • Maintain documentation regarding persons living with ALS/PLS in the Chapter’s database
  • Provide persons living with ALS/PLS, and their caregivers and families with community, state and national resources
  • Facilitate, at a minimum, one connection group and fill in for other Connection Group facilitators as needed
  • Attend assigned clinic(s) and participate in clinic discussion regarding clinic attendees (where and when offered); collaborate and coordinate with the Clinic Coordinator and multidisciplinary team as needed
  • Meet with patients at ALS clinic, either in person on clinic day or through follow-up phone calls or emails, to ensure that each clinic attendee and their families are knowledgeable about the disease and about available

Chapter services

  • Advocate and educate the North Carolina community regarding ALS public policy issues and efforts
  • May include the development, implementation and management all National Public Policy issues, including (but not limited to): coordinate logistics for Public Policy Conference, inform staff of action items and other pertinent information

Other Duties:

  • Identify needs and help develop potential additional programs to aid ALS families
  • Assist in the development of ALS/PLS educational related materials
  • Maintain and display ALSA materials in clinics
  • Provide In-Services to professional groups needing ALS information and education
  • Choose families that the employee feels will benefit from the Care Connection Program and train the team coordinators
  • Assist in development of the annual Outreach Professionals Conference
  • Assist in development of a statewide Caregivers Conference
  • Attend and participate in Care Services & Advocacy Strategic Planning
  • Be available during the months of March, April, May, October & November, and designated Saturdays and other night and weekend work as assigned, including (but not limited to) attendance at all Walks to Defeat ALS®
  • Support the Special Events Team on Walk Day by owning your designated area, which includes (but is not limited to): training volunteers, registration of participants/volunteers, set up or break down of areas, handling money or credit card donations, etc.

Care Services Social Worker 1

  • Attend and support non-Walk related events representing the Chapter in an advisory, lead or support role as needed
  • Attend and participate in all appropriate and/or assigned database trainings (Convio/Luminate & Donor

Pro/Salsa Labs) as applicable to primary & secondary job duties

  • Attend local, regional and/or national professional development trainings and conferences, including (but not limited to): National Public Policy Conference in Washington, DC, as assigned
  • Follow all assigned policies, processes & procedures
  • Update or create new departmental policies, processes & procedures as needed
  • Cross train in all secondary assigned duties
  • Retain & discard documents as appropriate per the Chapter Document Retention Policy
  • All other duties as assigned

Education/Experience:

  • Bachelor's Degree in Social Work required, Masters of Social Work preferred (A combination of relevant work experience and a degree in a related field will be considered)

Language Ability:

  • Able to read, analyze and interpret general business periodicals, professional journals, technical procedures and governmental regulations
  • Able to write reports, business correspondence and departmental procedures

Math Ability:

  • Able to compute rate, ratio and percent and to draw and interpret graphs

Computer Skills:

  • Strong knowledge of the Microsoft Office Suite
  • Familiarity with database software (Donor Pro/Salsa and Convio/Luminate experience a plus)

Certificates and Licenses:

  • Valid Driver's License

Supervisory Responsibilities:

  • Volunteers

Work Environment:

  • Generally, this job is performed in an indoor office environment
  • While performing the duties of this job, the employee is occasionally exposed to outdoor weather conditions

Physical Demands:

  • The employee must occasionally lift and/or move up to 20 pounds
  • While performing the duties of this job, the employee is regularly required to sit
