Overview:
To apply case management strategies to assess, plan, implement, coordinate, monitor and evaluate options and services to meet an individual's healthcare needs, through communication and the use of available resources to promote quality, cost effective outcomes.
The case manager, in close collaboration with the physician, will be responsible for the in-patient length of stay (LOS), denied days, resources utilization and will oversee the discharge planning process.

License Requirements: RN • Certification Requirements: N/A • Age Specific Care Provided For: N/A • Knowledge, Skills and Abilities: Computer proficiency and technical aptitude with the ability to utilize one or more of the following: ••• Microsoft Word, Power Point, Excel ••• Microsoft Outlook ••• Meditech (if applicable to position) ••• Epic (if applicable to position) ••• Performance Manager/NetLearning Must be able to read, write, and speak the English language in an understandable manner. Must be able to work independently and possess the ability to make independent decisions when circumstances warrant such action. Must possess the ability to effectively communicate and deal tactfully with other personnel, visitors, patients, physicians, customers, and the general public. Must be flexible, have personal integrity, and the ability to function effectively as a team with other employees. Must be able to follow written and oral instructions. • Other Requirements: Strong problem-solving and organization skills Must be able to accurately type 35 words per minute Function in a fast paced environment
Qualifications:
EDUCATION AND WORK EXPERIENCE
  • Education and Work Experience:
    • Must possess a high school diploma or GED
    • Must possess a degree in Nursing
    • 3 years in the clinical setting; Medical/Surgical experience preferred
    • Managed care experience preferred
    • Case management experience preferred
    • Utilization review skills using Interqual Criteria preferred
