JOB DESCRIPTION
Your Work

In this role, you will:

  • Establishing trusting relationships with members while providing general support and encouragement.
  • Conduct social determinants of health needs assessment and/or augmented information obtained from the social determinants screening tool.
  • Provide referrals and linkage to community agencies.
  • Provide in-person, field-based, and telephonic ongoing follow up and support to members accessing community agencies and services.
  • Establish strong relationships with community partners.
  • Actively participates with the interdisciplinary team and provides updates on members’ ongoing needs and progress with goals.
  • Actively participates in weekly or bi-weekly case consultations with a Master of Social Work (MSW) or Licensed Clinical Social Worker (LCSW), to support best practices for member engagement and case management.
  • Prepares and updates tracking and productivity reports, as needed.
  • Documents case notes in the clinical case management documentation system within the established guidelines.
  • Perform other duties, as required.

QUALIFICATIONS
Your Knowledge and Experience

  • Requires Bachelors Degree in Social Work or equivalent field.
  • Requires at least 3 years of prior relevant experience working with populations impacted by health and social issues.
  • Requires A minimum of 1 year experience navigating, linkage to, and interacting with community-based agencies and governmental systems.
  • Required Valid CA Driver’s License and proof of auto insurance coverage required.
  • Required proficient skills with computer programs, including Microsoft Excel, Outlook, Word, and PowerPoint.
  • Required excellent communication skills both orally and in writing with all levels of BSC Promise staff, members, contracted physicians, and participating provider groups.
  • Required knowledge of health disparities in diverse communities.

  • Required Effective time management skills and ability to define and act on priorities efficiently
  • Desired knowledge of governmental and private organizations, resources in the community, laws, regulations, and policies.
  • Desired experience and or training in motivational interviewing.
  • Desired Experience working with Medi-Cal populations.
  • Desired Bilingual/Spanish speaking.

Pay Range:

The pay range for this role is: $ 65083.70 to $ 91116.30 for California.

Please note that this range represents the pay range for this and many other positions at Blue Shield that fall into this pay grade. Blue Shield salaries are based on a variety of factors, including the candidate's experience, location (California, Bay area, or outside California), and current employee salaries for similar roles.

Physical Requirements:

Office Environment - roles involving part to full time schedule in Office Environment. Due to the current public health emergency in California, Blue Shield employees are almost all working remotely. Based in our physical offices and work from home office/deskwork - Activity level: Sedentary, frequency most of work day.
