Amazing Care is seeking a highly motivated, experienced RN to for the role of Home Health PDN Assistant Director in our Aurora office. This position will assists with organizing and overseeing all PDN services being provided. Will work collaboratively with all departments to provide high quality care in a safe environment. This position adheres to all Federal, State, Local and Company policies and regulations. Our mantra is simple: Let compassion and quality of service lead the way.

Job Responsibilities:

  • Develop relationships with the client's care team to include the physician, ensuring that orders are obtained, and care needs are clear with all involved
  • Assisting with oversight of the collective care team to achieve common goals for the client
  • Maintain visibility within the community to establish and connect with community partners, developing and building on relationships
  • Coordinate and take part in strategic meetings to promote increasing success of the department
  • Complete reassessments of client's needs and report back to necessary members of the client's care team as applicable
  • Maintain compliance of medical records and documentation of all assigned clients
  • Consult with physician or community partners to achieve client goals
  • Ensure understanding of PDN needed by all involved with the extension of training as required
  • Assist in the collection of necessary authorizations to provide required care of client and ensure reimbursement
  • Monitoring Patient Care, Assigning Clinicians, Coordinating Care, Coordinating Referrals, Assuring patient needs are continually assessed, assuring plans of care are developed, implemented, and updated

Requirements:

  • This position requires a Colorado RN license in good standing and a minimum of two years of management in home health.
  • Ability to apply and analyze data as it pertains to quality and compliance metrics
  • Comfortable working collaboratively in cross-functional teams
  • Ability to communicate effectively, both verbally and in writing, with all levels of an organization
  • Excellent presentation skills
  • Able travel on a regularly basis
