Responsibilities

PRN- Evening and weekend availability

The Pharmacist is responsible for coordinating, implementing, and monitoring pharmacy services. This pharmacist serves as a clinical resource to staff and serves as the pharmacy representative for multi-disciplinary process improvement. The Pharmacist is responsible for the ongoing development, implementation, quality management/monitoring, regulatory requirements and review of Pharmacy Services. The Pharmacist will serve as a resource to other health care professionals to provide effective, high quality, and safe patient care.

  • Participate in all aspects of CHI Memorial’s comprehensive clinical pharmacy services including rounds with medical teams, pharmacokinetic consultations, parenteral nutrition consultations, anticoagulation management, antimicrobial stewardship monitoring, medication reconciliation, discharge counseling, documentation of clinical interventions in the identified data base, and compliance with clinical pathways and protocols
  • Process prescriber orders in the identified pharmacy information systems in an effective, efficient, timely, safe and accurate manner to ensure the appropriateness of the indication, dose/frequency, route, interactions, dose adjustment, physical and therapeutic incompatibilities, side effects, duplicate therapies, and cost-effectiveness.
  • Evaluate need for all non-formulary medication required.
  • Provide direct oversight and timely and accurate verification of final product for all aseptic parenteral and non-parenteral medication preparation, dispensing and distribution in compliance with state, federal statues and hospital procedures.
  • Ensure doses are filled and delivered within established departmental guidelines in a timely and safe manner.
  • Identify and evaluate adverse drug reactions and medication errors
  • Provide enhanced communication and drug information to healthcare workers and patients
  • Provide teaching, supervision, and serve as a role model to technicians, residents, and students
  • Participate in professional activities within the department and in the community
  • Other duties as assigned by management

Qualifications

  • Graduate of an Accreditation Council for Pharmacy Education (ACPE) approved College of Pharmacy with a BS, MS, or Pharm D degree.

* TN Board of Pharmacy License. Licensure must be received within 90 days from the first day of employment. * Active ACLS * Board Certification by Board of Pharmacy Specialties (BCPS) in Critical Care or Pharmacotherapy

* Completion of ASHP-accredited PGY-1 Residency and/or 3 years of hospital experience as a clinical pharmacist. * 1 year experience as a PGY-1 preceptor, fulfilling ASHP preceptor requirements * Knowledge of the principles, practices, standards and techniques relating to the compounding and dispensing of medicinal drugs. * Knowledge of regulatory requirements applicable to the pharmacy profession. * Ability to use automated systems and office equipment at an acceptable level of proficiency. * Knowledge of the principles and practices of pharmaceutical care, including IV fluids, IV additive programs, automated dispensing systems, and IV therapy services. * Ability to convey information effectively in verbal and written communication. * Computer applications such as Microsoft Office * Excellent time management and communication skills
