Wound Care Nurse - Full-time, Days - Bronson Commons, Mattawan
The Wound Care Nurse (WCN) provides wound care services and nursing support to patients and residents at Bronson Commons. The WCN provides wound care consultative support to the nurses providing direct patient care and provides basic continuing education on the prevention and management of wounds to nursing staff.
High school diploma or general education degree (GED); 2 years formal education/training, or Associate's degree; wound care experience preferred
RN in good standing with the State of Michigan required
Wound Care Certification required within 12 months of hire

Work which produces levels of mental/visual fatigue which are typical of jobs that perform a wide variety of duties with frequent and significant uncontrollable deadlines. Work may include the operation of and full attention to a personal computer or CRT up to 40 percent of the time. Involves regularly lifting of bulky or moderately heavy weights (i.e., up to 50 pounds), and occasionally assisting with heavier tasks or expending the equivalent effort in pushing, pulling, or otherwise handling material, equipment, and other objects.
  • Demonstrates competent use of the Nursing Process, application of wound/hyperbaric theory, technical skills, and critical thinking in the delivery of holistic, culturally sensitive, age and developmentally appropriate care to patients served within the assigned unit; and other clinical areas as needed.
  • Collaborates with patient, family, and significant others; and other disciplines for comprehensive care planning.
  • Performs comprehensive patient assessments, anticipating potential care problems, and prioritizing care needs in order to effectively provide optimum care.
  • Initiates and communicates change in patient care delivery based on continual evaluation of patient progress according to outcome-based care plan.
  • Coordinates discharge and outpatient plan of care related to wound care with patients and families
  • Collects data to analyze outcome-based care management reports to evaluate patient progress.
  • Prepares data to actively participate in weekly Clinical Oversight meetings to discuss wound care with Interdisciplinary Team
  • Delegates appropriate tasks, monitors and maintains accountability for the quality of care.
  • Develops and coordinates the education plan appropriate to the patient/significant other/family learning style.
  • Communicates plan to patient/significant other/family and/or interdisciplinary care team members
  • Demonstrates flexibility in scheduling to align with facility and patient need.
  • Participates in wound care referral reviews upon request
  • Manages resources, which promote cost effectiveness, efficient operation of unit, and positive outcomes.
  • Demonstrates critical thinking and independent decision-making utilizing policies and procedures, standards of care, and data analysis (e.g., manages crisis prevention and intervention at a unit level.)
  • Demonstrates commitment to providing outstanding customer service in a manner that is reflective of our mission, values, and customer service standards.
  • Maintains a working knowledge of applicable federal, state and local laws and regulations, the Compliance Accountability Program, Code of Ethics, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior.
  • Maintains a safe workplace by keeping work area free of hazards and correcting and/or properly reporting any safety hazards.
  • Uses correct body positioning, proper body mechanics to avoid injury, utilizes safe work practices (e.g. pushing vs. pulling, patient transfer/lifting devices, etc.) and uses personal protective equipment when necessary.
  • Performs related duties as assigned.
  • Utilizes an evidenced based practice focus in the development and utilization of standardized care plans, clinical pathways, guidelines/protocols, policies and procedures.
